The two look identical, are packaged similarly, and are both described as sterile. The difference is one ingredient, and it changes how the vial behaves for the rest of its life.

What "bacteriostatic" means
Bacteriostatic describes something that inhibits bacteria from multiplying, as distinct from bactericidal, which kills them. Benzyl alcohol at around 0.9% is bacteriostatic. It does not sterilise the vial, which was already sterile; it maintains that state after air and a needle have been introduced.
This is the whole reason multi-dose vials are viable. Every entry through the stopper is an opportunity for contamination; the preservative is what keeps a small opportunity from becoming a growing one over the following weeks.
Where the 28 days comes from
Manufacturer labelling for bacteriostatic water commonly specifies an in-use period of up to 28 days from first entry. That figure describes the preservative system in the water, not the peptide dissolved in it. The compound has its own stability characteristics, which may be shorter, and which come from its own labelling.

Why sterile water is single use
With no preservative, an opened vial of sterile water has nothing inhibiting growth in whatever remains after the first draw. The convention of discarding it is not caution for its own sake; it follows directly from the composition.
When the choice is not free
Benzyl alcohol is not appropriate in every context. There are populations and clinical situations where preservative-containing diluents are avoided, and there are individual sensitivities. Which diluent is appropriate for a given person and a given product is a question for your healthcare professional. It is not a preference to be settled by convenience.
Does the diluent affect the math?
No. Concentration is vial strength divided by diluent volume, and the arithmetic is identical whichever water was used. 5 mg in 2 mL is 2.5 mg/mL regardless. Common points of confusion
- "Sterile water is the purer option." Both are sterile. Only bacteriostatic water stays protected once opened.
- "The 28 days runs from when I bought it." It typically runs from first entry into the vial.
- "Bacteriostatic water sterilises the peptide." It does not sterilise anything. It inhibits growth in an already-sterile solution.
- "More preservative would last longer." The concentration is a formulation decision, not a dial.
